InTouch Family Connection Program
Keeping patients connected with loved ones is more important than ever to their physical and emotional well-being. The St. Croix Hospice InTouch Family Connection Program demonstrates our commitment to helping patients and their families experience the hospice journey together, even when they’re apart. Staying connected provides support, reassurance and peace of mind when it is needed the most.
The InTouch Family Connection Program tools include:
Video Chats. We use technology such as Google Duo and Facetime to connect patients with loved ones during our visits. We can help families install these apps if needed.
Phone Calls. Recognizing families want to know about their loved one’s health when they are apart, our teams check-in often by phone. We also make phone calls during our visits so patients and family members can talk directly.
In-Person Visits. We coordinate in-person visits with patients, families and facilities when possible, including providing PPE (personal protective equipment) to keep everyone safe.
The St. Croix Hospice InTouch Family Connection Program is led by our Social Workers and Chaplains with involvement from our RN Case Managers and Hospice Aides.
